Please share our 'Mind Your Wellness Survey 2.0'
Help us collect 3,200 responses!
We are excited to launch 'Mind Your Wellness Survey 2.0' and need your help to collect responses!

This survey provides an in-depth look into the mental health of adults in the Fox Cities and is used to inform community recommendations for prevention and support.

We first launched this survey three years ago, which informed the report "Mental Health & Suicide Prevention Recommendations," released in November 2022.

Since that time, we've made progress, and the revised survey is designed with cultural considerations in mind to ensure mental health equity for all.

We invite you to share this survey far and wide with your networks, to ensure we reach a diverse and representative sample of adults.

Here's how you can help us spread the survey:
- Print out copies of the flyer. Display in your workplace!
- Share on social media and/or your agency's website! Click below for graphics and links to the survey. The survey is available in English, Hmong and Spanish.
