Project Zero | Mind Your Wellness Survey

The Mind Your Wellness Survey, an initiative of Project Zero, is a local survey conducted in Calumet, Outagamie and Winnebago counties to gain a better understanding of adult mental health and suicide-related behaviors. The survey, designed by the Medical College of Wisconsin, was first administered in 2021 and provided a detailed look at risk and protective factors related to suicide and the mental health of adults living in the Fox Cities. The survey results informed the special report Mental Health & Suicide Prevention Recommendations, published in 2022, which features dozens of recommendations for our community to improve adult mental health and reduce adult suicides in the tri-county region.

In August 2024, Project Zero launched an improved upon and culturally-informed Mind Your Wellness Survey 2.0, to measure progress made since the first survey came out in 2021. The project closed the survey in August 2025, and preliminary data analysis will be shared at our November 13, 2025 member meeting.  In early 2026 we anticipate publishing an update on the recommendations report as well as data from the original MYWS and MYWS 2.0.
