Project Zero Adult Suicide Death Review
Summary Report, 2026
The Tri-County Adult Suicide Death Review 2022-2025 Summary Report highlights a collaborative regional effort to better understand suicide deaths and strengthen prevention across Calumet, Outagamie, and Winnebago counties. Developed through Project Zero, an initiative of the Northeast Wisconsin Mental Health Connection, the report shares local suicide data, key findings from multidisciplinary adult suicide death reviews, and prevention recommendations designed to improve systems, close service gaps, and save lives.
Healthy Teen Minds
Summary Report, 2024
The Healthy Teen Minds Report highlights an eight-year (2016–2024), community-wide effort to improve youth mental health across Calumet, Outagamie, and Winnebago counties. The initiative focused on upstream prevention, early intervention, and improving access to care. Key strategies included the region-wide implementation of Sources of Strength, an evidence-based school program that empowers students to build resilience and seek help, and the creation of MyConnectionNEW.org, a mental health resource navigation website. Together, these efforts contributed to positive shifts in local data trends and strengthened a more coordinated, prevention-focused approach to supporting youth mental health in the Fox Valley.

Mind Your Wellness Survey Report, 2023
The Mind Your Wellness Survey Report (2021) highlights a first-of-its-kind, regional effort to better understand adult mental health and suicide risk across Calumet, Outagamie, and Winnebago counties. Developed through Project Zero by the Northeast Wisconsin Mental Health Connection in partnership with the Medical College of Wisconsin, the survey gathered population-level data on mental health, risk and protective factors, and suicide-related behaviors among more than 1,200 residents. The report shares key findings and community-informed recommendations focused on basic needs, access to care, and social connection, providing a data-driven foundation to guide systems change and strengthen suicide prevention efforts across the Fox Valley.

The 10-Year Anniversary Report highlights a decade of collaborative, community-driven efforts to improve mental health and well-being across Calumet, Outagamie, and Winnebago counties. The report reflects on key milestones, initiatives, and partnerships that have shaped the region’s approach to mental health, including Project Zero, Healthy Teen Minds, and MyConnectionNEW.org. It also shares lessons learned and illustrates how sustained cross-sector collaboration has strengthened systems, increased awareness, and advanced a more connected and responsive approach to supporting mental health across the Fox Valley.

Community Health Report, 2025
The Tri-County Community Health Report (2025) highlights a first-of-its-kind, collaborative effort to understand health and well-being across Calumet, Outagamie, and Winnebago counties. Developed by the Tri-County Community Health Improvement Coalition, the report brings together shared data, community voice, and cross-sector insight to identify key drivers of health, including mental health, belonging, and the conditions in which people live, work, and connect. Grounded in the Vital Conditions for Health and Well-Being framework, it provides a unified foundation for regional alignment and coordinated action to improve health outcomes across the Fox Valley.
