Strategy Hub
The Strategy Hub below is an interactive, living inventory of programs, strategies, and initiatives that support social connection and belonging across Calumet, Outagamie, and Winnebago counties. It is designed as a practical tool for our community to better understand what is already in place, what is emerging, and how different efforts fit together.
The purpose of the Strategy Hub is to help our region take stock of the many ways social connection is being fostered through existing organizations, projects and programs. By making existing and emerging strategies visible in one place, the Hub highlights the range of solutions already happening in our communities, surfaces innovative ideas, and supports greater alignment across sectors. Seeing what is underway helps reduce unnecessary duplication, reveals overlaps, and points to gaps and opportunities where additional investment or coordination may be needed.
How the Strategy Hub is Organized
This hub organizes strategies that strengthen social connection and belonging across the Fox Valley. It combines two complementary frameworks:
- the Vital Conditions for Health & Well-Being (systems-level conditions)
- the Six Points of Connection (everyday relationship-building)
Together, they help show both where connection is built and how it happens.
How the Strategy Hub Works
System-Level Conditions
The first layer of the framework is grounded in the Vital Conditions for Health & Well-Being. These are the community conditions that help people thrive. They include:
- Belonging & Civic Muscle
- Basic Needs for Health & Safety
- Humane Housing
- Meaningful Work & Wealth
- Lifelong Learning
- Reliable Transportation
We have added two additional systems-level categories:
- Health & Wellness – .Recognizes that healthcare and wellness systems can play an important role in fostering connection and reducing isolation.
- Built Environment – Recognizes that the design of neighborhoods and public spaces influences how people gather, interact, and build relationships.
These conditions shape whether people have opportunities to connect, participate, and feel supported in community life.
Interpersonal Connection
The second layer uses the Six Points of Connection, developed by the U.S. Chamber of Connection. These describe the everyday ways people build relationships and social trust.

- Community Service
Using this framework, Connected Fox Valley inventoried existing programs, initiatives, and strategies to better understand how they strengthen social connection and belonging at both the systems and interpersonal levels. This approach helps reveal how many community efforts may contribute to connection and well-being, even if social connection is not their primary focus.

The Strategy Hub is still under development and will continue to grow over time – we are just getting started populating it with all the programs and services already in place that support social connection. Additional programs, strategies, and initiatives will be added as Connected Fox Valley continues its research, listening sessions, and community conversations.
